Registration and Appointment Check-In

Here at Union General Rural Health Clinic we are always working to enhance and streamline your office visits. Our pre-registration and pre-visit check-in software, Phreesia, assists us in giving you the best experience possible. We are ditching the clipboard of paperwork and opting for the newest, secure, and digital solution. This system helps us to obtain the most up to date information, reduce our paper consumption, and reduce time spent in the waiting room for registration, therefore reducing wait time for seeing your provider. Here’s how it works-

After scheduling your appointment via our call center or online booking tool, you will receive a text message up to 3 days in advance from Phreesia. This message will contain a link asking you to verify your information.

  • New Patients: You will be asked to fill out your demographic information and medical history. After completing this, please review and sign our HIPAA, Privacy, and Patient Policies. If your appointment requires a copay, you can pay that ahead of time!
  • Existing Patients: You will be asked to verify information already in our system, update anything that may have changed, and sign our HIPAA, Privacy, and Patient Policies if it has been over one year since you signed them last. If your appointment requires a copay or you have a balance, you can pay that ahead of time!
Online Check In

After completing these steps, please arrive 10-15 minutes early to your appointment and let the receptionist know you’re here for your appointment. If you are a new patient, please come with a form of identification and your most up to date insurance card (if using insurance).

If you are unable to complete the pre-visit registration/check-in, please come 15-20 minutes early to your appointment. You will be sent a link to your device in office or given one of our Phreesia Pads to complete your registration.


Q: What is Phreesia?

A: Phreesia gives healthcare organizations a suite of applications to manage the patient intake process, including mobile registration. Mobile check-in allows you to complete pre-visit registration from your own device (any smartphone, computer or tablet) ahead of time and in the privacy of your home.

Q: Is the Phreesia system secure?

A: Yes. Phreesia provides industry-leading privacy and security for our patients’ data. They are held to the same standards as Physicians & Midwives related to protecting your family’s information. For additional information about Phreesia’s security visit https://www.phreesia.com/patient-privacy.

Q: Do we need to use the new system for every appointment?

A: Yes. But once you have completed the initial registration, the next time will be much quicker because Phreesia saves your answers.

Q: Do I need to download an app to do mobile check-in?

A: No. There is no app – you simply click the link provided in the text message or email. This allows you to securely answer any registration and medical questions necessary for your appointment.

Q: Can I complete the forms in a language other than English?

A: Yes! Mobile registration is available in 20 different languages, and the PhreesiaPads in the office offer Spanish in addition to English.

Q: What happens if I can’t finish my registration before the appointment?

A: That’s okay! If you have not completed your registration before the visit, see one of our receptionists, and they will be happy to send you a new link. Don’t worry, Phreesia saves information you entered previously.
